What is my Ultipro login for employees? ›

Username: Your username is your last name, first initial, and your employee ID number. Default password: your date of birth. Click the Log In button.

Through MyChart, you can schedule appointments, access test results, communicate with your doctor, request prescription refills, tell an Emergency Center or Urgent Care you're on your way, and more.
